Telegraph Speech and Debate’s:
Policies and Procedures for the 2020 Summer and/or 2020-21 School Year
ASSUMPTION OF RISK, RELEASE AND WAIVER OF LIABILITY AND INDEMNITY AGREEMENT
I do hereby declare myself and my son/daughter to be physically sound and suffering from no condition, impairment, or other illness that would prevent my and my son’s/daughter’s participation in the activities provided, sponsored and/or attended by Telegraph Speech and Debate.
I do further hereby acknowledge and understand that Telegraph Speech and Debate does not carry or maintain health, medical or disability insurance for the undersigned and therefore agree to assume responsibility for such insurance coverage on the undersigned.
I understand that there may be inherent risks, dangerous conditions, or harmful consequences of my attendance at, and participation in tournaments, events, classes, and competitions.
I acknowledge that there are both known and unknown risks in the activities provided, sponsored and/or attended by Telegraph Speech and Debate and that there may be unavoidable risks involved in such activities.
I hereby acknowledge and agree that I understand the nature the activities provided, sponsored and/or attended by Telegraph Speech and Debate; and that my son/daughter is qualified, in good health, and in proper physical condition to participate therein.
I understand that if a medical emergency does arise with my son/daughter, Telegraph Speech and Debate, its officers, employees, volunteers, and agents will not take any responsibility to treat your son/daughter, but will prompt contact 911 emergency services; Telegraph Speech and Debate, its officers, employees, volunteers, and agents are not responsible for any medical expenses.
I, my heirs, executors, administrators, or assigns hereby waive, release, and forever discharge in advance Telegraph Speech and Debate, its officers, employees, volunteers and agents from any and all liability, actions, causes of actions, damages, claims for damage for personal injury, death, property damage, or demands, which I may have, or which may hereafter accrue to me, whether such injuries are caused by my negligence or the negligence of Telegraph Speech and Debate or any of its officers, employees, volunteers and agents, arising out of or connected in any way with my participation in the activities provided, sponsored and/or attended by Telegraph Speech and Debate.
Also, I agree to indemnify Telegraph Speech and Debate, its officers, agents, and employees and to pay for any costs, attorney fees, or awards that may result from resisting any complaint or lawsuit which I may bring against the above-named parties for any injury or loss I claim to have suffered.
PAYMENT, POLICIES, AND PROCEDURES
I acknowledge that I am registering for the 2020 August Drills-only class that lasts one month (based on the schedule listed on telegraphsd.com).
I understand that it is my child's and mine's responsibility to arrive at class or log in online on time and prepared with all the necessary material and completed assignments. If a student misses a class it is his/her's and the parents' responsibility to make sure all of the work for the next class is complete.
I understand that due to safety concerns the premises of Telegraph Speech and Debate's offices/online classes are being recorded; and my child, myself, Telegraph Speech and Debate's staff, and anyone else who walks into the vicinity is being recorded.
Make Up Class Policy: there are no make up opportunities for the Drills Class, but recordings of the drills will be available on telegraphsd.com
While video recordings will generally be available, Telegraph Speech and Debate can’t anticipate the various technical problems that may prevent a video from being uploaded. I understand that while video recordings will generally be available, occasionally those will be replaced with written class summaries.
Furthermore, I understand that if my child would like to attend additional classes, I must pay an additional fee added to the tuition.
Students and Parents agree to comply with all of Telegraph Speech and Debate's policies and procedures stated here and introduced during the period that the child participates in the program.
Telegraph Speech and Debate staff and program directors hold the right to deny any student the right to attend classes if the said student's behavior does not comply with Telegraph Speech and Debate's policies and procedures.
I understand that upon enrollment I am responsible to pay the entire amount listed on the registration page. All tournament fees are paid separate upon tournament registration. Telegraph Speech and Debate does NOT give refunds for any payment (school year tuition nor tournament fees). For certain classes – if advertised – refunds will be given if requested within the first two class sessions.
I understand that if I register my child for a tournament, I chaperone him/her, as Telegraph Speech and Debate staff is not responsible for the safety of my student.
AUTHORIZATION FOR THE USE OF CHILD/YOUTH NAME, LIKENESS, AND/OR PHOTOGRAPHIC IMAGE
This authorization form shall serve as parental permission for the use of name, likeness, and/or photographic image of a child/youth where such permission is required.
I grant permission to Telegraph Speech and Debate to use my child’s/youth’s name, likeness, audio recordings, video recordings, and/or photographic image in the production of the following: TV shows, websites, brochures, promotional pictures, other promotional material, newspapers, online and print publications, etc. I understand that if, for whatever reason, at any point in time, I decide to revoke this agreement, and I so notify Telegraph Speech and Debate in writing, all references to my child/youth (i.e., name, likeness, audio recordings, video recordings, and/or photographic image) will no longer be used. I understand that web page references and web page photographic images will be removed within thirty (30) days of the written notification. I understand that Telegraph Speech and Debate is not responsible for access to the internet information or downloads made by users using the web prior to this removal of web references (i.e., name, likeness, audio recordings, video recordings, and/or photographic image). I further understand that my child’s/youth’s name, likeness, and/or photographic image may continue to be used in any publication already printed or published prior to my revocation of the consent provided herein.
SPEECH AND DEBATE TOURNAMENT(S) POLICIES
I hereby grant permission for my daughter/son to participate in speech and debate tournament(s) and their designated venues as a part of Telegraph Speech and Debate's Speech and Debate Team.
I understand that for certain tournament(s) I, myself or my spouse will be asked to judge for my daughter/son for the duration of the tournament. Failure to provide a judge or purchase one (a $100 fee upon registration) will result in the dismissal of your daughter/son from the said tournament.
I understand if I, my spouse, or another designated party that is registered as a judge for my daughter/son’s entry fails to show up to his/her assigned round(s) I will take on full responsibly to pay any fine the tournament will administer for the missed round(s).
I understand that I, myself or my spouse is required chaperone my daughter/son for the duration of the tournament. I also understand that if I choose to leave my daughter/son unchaperoned for the duration of the tournament that Telegraph Speech and Debate, its officers, employees, volunteers and agents are not responsible for any injuries or property damage sustained by my daughter/son.
If the student attend the tournament in a physical location then:
I agree to provide transportation for my daughter/son to and from the tournament(s)' designated venues; Telegraph Speech and Debate, its officers, employees, volunteers, and agents will not provide transportation for any student. Also I understand that my daughter/son is not allowed to leave the premises of the tournament(s)'s venues for the duration of the tournament(s) without the supervision of me, my spouse, or another designated party.
I agree to direct my child to cooperate and conform with the directions and instruction of supervisory personnel in charge of the tournament. Should it be necessary for my daughter/son to have medical treatment of any kind while participating in the tournament, I hereby give the tournament supervisory personnel full rights in obtaining medical service for my child and I give tournament supervisory personnel permission to render medical treatment deemed necessary and appropriate.
